I acknowledge I volunteer my services to the City of Elmhurst and I am not an employee of the City of Elmhurst. I agree that my services are donated to the Elmhurst History Museum without compensation.

I understand that my volunteer status may be terminated at any time for failure to comply with policies and procedures of the Elmhurst History Museum as outlined in the Elmhurst History Museum Volunteer Manual.
